About This Exciting Opportunity We're seeking a dedicated Care Coordinator to join our Personalised Care Team,
specialising in safeguarding support for children, young people, families, and vulnerable adults. This role is integral
to our mission of providing coordinated, patient-centred care whilst ensuring the safety and wellbeing of our most
vulnerable patients. What Makes This Role Special As our Care Coordinator (Safeguarding), you'll be at the heart of
ensuring our most vulnerable patients receive timely, coordinated care. You'll work across our seven GP practices,
